Revision Penile Implant for Floppy Glans Penis
Case Presentation
A 53-year-old man presented to our clinic with a 4-year history of a flaccid glans penis. His past medical history included a penile fracture in 2017 that was corrected surgically. After undergoing this procedure, he developed erectile dysfunction, and in 2021 a SHAH penile implant was placed. (b) The patient has a treated history of hypertension and diabetes mellitus for the past 15 years.
Floppy glans syndrome: what is it?
Floppy glans is a possible complication after incorrect placement or sizing of a penile prosthesis. It may manifest as a side droop or hypermobile glans and causes discomfort and dissatisfaction with intercourse. The shaft may still work, the penis may get hard, but this is great and necessary — but if the glans don’t engorge well, aren’t responsive, depending on your partner, the sexual gratification you can get from your penis and your sexual life in quality of life changes significantly!
Diagnosis and Treatment Plan
The patient complained of dissatisfaction about his sexual life because of the floppy glans and looseness of the penis in general. On examination, he was found to have floppy glans syndrome secondary to the previous implant surgery conducted in 2021.

- Implant Type: SHAH WH 11
- Size: Right: 19.5cm; Left: 19.5cm
Considering the chronicity of the patient’s condition and dissatisfaction, we suggested a revision implant surgery with the Shah Indian Implant SHAH WH 13 with the following size details:
- Right: 22.5 cm
- Left: 22 cm
Surgical Outcome and Patient Satisfaction
The patient experienced a marked improvement in satisfaction after revision surgery. The new implant fit better, and offered more support and stiffness to the glans, addressing the floppy glans problem and restoring erectile function.
In addition to physical benefits, the patient encountered psychological relief, as the frustration over floppy glans was removed. Post-op, his confidence and sexual satisfaction markedly increased.
Conclusion
Floppy glans syndrome genitourinary effects can greatly affect a patient’s sexual health as well as their overall well-being. Furthermore, correct penile implant choice and positioning is key to success. We report a case that emphasizes the role of revision surgery to salvage complications and enhance overall satisfaction.
